Important Information About Your Consultative Examination for Disability Determination Services

If you or your child have been scheduled an in-person appointment for a consultative examination with Elizabeth Kauff, MA by NC Disability Determination Services, the following is important information about your appointment. Please read the information carefully and be sure to follow the instructions provided.

Before Your Appointment:

You should receive a text with a link to a background questionnaire that should be completed BEFORE the day of your scheduled appointment. It can be completed by text or an email link can be sent for the form to be completed on a desktop computer. The form does not need to be printed. You will submit it online and it will be printed at the office. If you are unable to complete the form online or with the text link, the form will be provided for you to complete with paper and pen at the appointment.

What to Bring to Your Appointment:

1. Bring ALL of your medications with you, as well as vitamins, supplements, and over-the-counter medications you take.

2. Bring your eyeglasses if you need them to read or see things up close.

3. Bring your hearing aids if you have any difficulty hearing.

4. Bring a photo ID (driver's license, state-issued ID card, student identification card, or something similar).

5. Bring any paperwork or records that you feel the provider may find helpful. Only bring mental health, previous testing, or educational/school records. We are not able to evaluate or assess any physical or health complaints you have at this appointment.

6. Bring a list or the name(s) of your doctor/treating physician(s) and the names of any places you received mental health treatment in the past.

NOTE:

1. If you (or your child, if the child is the person being evaluated) take prescription medication(s) on a typical day or a school day, please TAKE THE SAME MEDICATION(S) AS PRESCRIBED ON THE DAY OF YOUR APPOINTMENT. The provider does not need to "see how he/she acts" without the medications. If the client is unable to cooperate during the appointment, the appointment will be canceled.

2. If a child is scheduled for the appointment, a PARENT/LEGAL GUARDIAN MUST ACCOMPANY THE CHILD TO THE APPOINTMENT. Do not have another family member or a friend bring the child without the legal guardian. The parent/legal guardian MUST be present in person to sign consent (give permission) for the child to be evaluated. The provider will not call the parent on the phone to obtain permission. If you are not the child's parent but have legal guardianship, please bring a copy of the court order showing your guardianship status to the appointment. IF THE PARENT/LEGAL GUARDIAN IS NOT PRESENT FOR THE APPOINTMENT, THE APPOINTMENT WILL BE CANCELED. There are NO EXCEPTIONS to this rule.

3. If you (or your child) wear glasses or hearing aids, you MUST wear them during the appointment. Testing is not valid and cannot be completed if the glasses or hearing aids are not worn.

4. Only ONE adult (person 18+ years old) will be allowed into the office for the appointment. Others who travel with you will have to wait outside. We respect your privacy and want you to feel that your evaluation is completely confidential.

5. If the appointment is for a child, only one adult (one parent/legal guardian) may come in the waiting area with the child. Please do not bring any other children with you, since there will not be anyone available to supervise them and we are committed to minimizing distractions. If other family members come to the office, they will need to wait outside in the parking area. 

6. If you (an adult) are scheduled for an appointment, do not bring your children with you. No one is available to supervise your child(ren) while you meet with the provider and your child(ren) will not be allowed to sit unattended in the waiting room or to be in the exam room with you during your appointment. Issues are often discussed with you during the appointment that are not appropriate to be discussed in front of a child. We want you to be comfortable and free of distractions during your appointment. If you bring your child(ren) to the appointment and no one is available to stay in the parking area with them for supervision, your appointment will be canceled.

7. Absolutely NO CELL PHONE USE is allowed in the office. This includes taking/making phone calls, messages, watching videos, or playing games.

8. Please do not bring any food or drinks inside the office.

9. If you need to cancel or reschedule your CE appointment, you MUST contact NC Disability Determination Services at (919) 814-3222 or 1-866-542-8113 to request a cancellation or to reschedule your appointment. While we appreciate you letting us know by phone call or text message if you are unable to attend your appointment, our office is unable to reschedule the appointments here.

10. Our office is NOT ABLE to release or send a copy of an evaluation done for Disability Determination Services to ANYONE except NC DDS. If you need a copy of an evaluation for any reason, you MUST contact NC DDS Professional Relations at 1-800-443-9360 and speak to a staff member who can assist you with your request.

On the Day of Your Appointment:

1. Please arrive 15 minutes early for your scheduled appointment. Since your appointment time is set aside especially for you, we will begin on time. If you arrive late for your appointment, the provider will NOT be able to meet with you and you will need to contact NC DDS to reschedule your appointment. There are NO exceptions to this rule. Your time is valuable and we want you to be treated with the respect you deserve. As such, we start on time and end on time.

2. When you arrive at the office, please text your FIRST NAME ONLY to 919-436-6152. You will receive confirmation that you have checked in and further instructions. 

3. Please plan to be at the appointment from 30 - 90 minutes, depending on the type of evaluation NC DDS has requested. We are happy to give you an estimate of the time the appointment will likely take when you arrive so that you will be able to give your transportation that information. Please do not have someone drop you off without a plan to be picked up promptly once your appointment is completed.​​​​​​​​​
